About Us Our organization has been at the forefront of establishing
the first national in-home clinical network. Today, we operate as
an independent provider of thorough health assessments, catering to
individuals nationwide. We are dedicated to delivering exceptional,
personalized care that enhances health, reduces costs, and empowers
individuals to lead fulfilling lives. With a strong foundation in
clinical assessment and care management, our extensive network of
clinicians transcends traditional barriers by meeting members in
their own environments. We aim to assist older adults and other
at-risk populations in achieving better care quality, improving
health outcomes, and identifying chronic conditions that might
otherwise remain undetected. Joining our team allows you to utilize
your skills and compassion to make a tangible difference in the
health and well-being of others. We offer competitive base salaries
along with incentive compensation and benefits, all while
contributing positively to the community! Responsibilities About
the Role During visits that may last up to one hour, our providers
assess and observe a member’s health status, medical history,
medication adherence, social circumstances, and other potential
risks. This approach provides invaluable insights into a member’s
overall health that are often missed during standard office visits.
The Matrix Comprehensive Health Assessment (CHA) enhances care
quality and enables us to address multiple care gaps in a single
appointment. Our providers are responsible for: Conducting
assessments for adults and geriatrics, including medical history,

line with our guidelines and protocols. Administering and ordering
necessary point-of-care and lab-based tests. Collaborating with
physicians, case managers, social workers, family members, key
caregivers, and other medical personnel as needed. Working
alongside Primary Care Physicians (PCPs) to provide patient
education and follow-up as required. Delivering services at the
member’s residence, which may include their homes or Skilled
Nursing Facilities. Benefits of Working with Us Consistency – While
assessments are conducted from Sunday to Saturday between 8 am and
7 pm, our full-time providers typically work Monday to Friday from
8:00 am to 5:30 pm. Competitive Pay – Enjoy an annual base salary

while conducting health assessments. Qualifications Skills &
