Job Description Job Description Position Summary: The Family
Nurse Practitioner (FNP) provides comprehensive, high-quality
primary care services to patients of all ages, focusing on health
promotion, disease prevention, and the management of acute and
chronic illnesses. Working as part of a collaborative care team,
the FNP serves as a key provider in delivering patient-centered,
culturally competent care. Key Responsibilities: Provide direct
patient care, including physical exams, diagnosis, treatment, and
follow-up for acute and chronic conditions. Order and interpret
diagnostic tests such as labs, imaging, and screenings. Prescribe
medications and develop treatment plans in accordance with state
and federal regulations. Educate patients and families on health
management, disease prevention, and treatment compliance.
Collaborate with physicians, nurses, behavioral health staff, and
other healthcare professionals to ensure comprehensive care.
Maintain accurate and timely documentation in the electronic health
record (EHR). Participate in quality improvement initiatives and
comply with clinic policies, procedures, and regulatory standards.
Provide care that is respectful of and responsive to the
preferences, needs, and values of diverse populations.
Qualifications: Master’s or Doctorate degree in Nursing from an
accredited program Current licensure as a Registered Nurse (RN) and
certification as a Family Nurse Practitioner (FNP-C or FNP-BC)
Current DEA license and prescriptive authority in the state of
practice BLS certification (ACLS preferred) Minimum of 1–2 years of
experience as an FNP in a primary care or similar setting preferred
Excellent clinical, communication, and organizational skills
Bilingual (English/Spanish) a plus but not required Work
Environment & Schedule: Outpatient clinical setting Monday–Friday
schedule with occasional extended hours as needed Minimal travel
between clinic locations (if applicable) Competitive pay and
