Acute Care (LTAC) hospital. We provide cardiopulmonary care,
ventilator and respiratory care, complex wound care, rehabilitation
therapies, addiction medicine program, geriatric psychiatry
program, multidisciplinary pain program, and outpatient counseling
program. With four locations in Shreveport and Bossier City, we
will be the largest provider of LTAC services in Northwest
Louisiana.Working at a LTAC facility like Intensive Specialty

TEAM.The LPN/LVN functions as a health care team member. The
LPN/LVN contributes to the assessment of the health status of
individuals and groups, participates in the development and
modification of the plans of care, implements care strategies
within the LPN/LVN scope of practice, renders safe care,
participates in the evaluation of responses to care interventions,
delegates activities within the LPN/LVN scope of practice, and
performs additional functions for which the LPN/LVN has been
trained within the professional scope of
practice.Responsibilities:Utilize nursing knowledge and skills in
the safe delivery of patient care through data gathering, planning,
and evaluation.Provides care based on physical, psycho/social,
educational, safety, and related criteria, appropriate to the ages
of patients served.Maintain and coordinate communication
effectively and professionally with all patients/significant
others, staff, and physicians to ensure a quality patient
experience.Maintain patient files, records, and other information
in a professional manner following policies and procedures
regarding documentation.Adapts to rapid change in a positive manner
throughout the day as needs of the unit/department
change.Requirements/Qualifications:Graduate of an accredited school
of practical/vocational nursingMust hold a current state or compact
state license and must maintain license renewal in accordance with
the standards of the State Board of NursingMental health experience
preferredExcellent organizational skillsGood time management skills
and self-directedDemonstrates good judgment, good interpersonal,
