Job Description:

A company is looking for a Transitions of Care RN - Bilingual Gujarati for a 100% remote position.
Key Responsibilities:

Provide patient outreach, triage, and disease education through telehealth modalities
Collaborate with healthcare providers to ensure patients receive necessary care
Participate in quality improvement processes and initiatives

Required Qualifications:

Fluency in Gujarati
Active, unencumbered, compact RN license
Experience in care of adult and pediatric patients with chronic illnesses
Utilization Management or Case Management experience preferred
Previous Transitions of Care experience is a plus
